Questionnaire

This questionnaire is tailored to families who need simple estate plans, which means it will only include the following documents:

​

• Basic Wills

• Advanced Directives: (Designation of Health Care Surrogate/Living Will)

• HIPAA Waivers

• Durable Power of Attorneys

• Designation of Health Care Surrogate For Minors –if applicable

• Estate Planning Binder with printed copies of all documents, instructions for signing, and tabs to assist with the execution process

• A trust may be added for an additional cost

If your answers to the questionnaire suggest that you may need a more complex estate plan, you will be contacted by one of our attorneys to discuss other options.

If you are married, please have each spouse fill out a separate questionnaire. It may take between 20 to 40 minutes to complete this questionnaire.
